Emergency Information

EMERGENCY FORMS

SCF developed an “In Case of Emergency Contact Form” to as­sist the church if contacted, who to contact in your extended fami­ly in case of an emergency.
All information is confidential.
If you do not have a form and would like one, contact Pastor Rob at the Church Office.
You can either pick up a copy at the church or have him send one as an e-mail attach­ment.

MEDICAL EQUIPMENT:
We have a storage locker of used medical equipment for loan to members and friends.
If you have medical equipment needs, please contact Gina Rosbrook at 541-593-2663 or ginarosbrook1@gmail.com.

MEDICAL TRANSPORTATION NEEDS:
Have a medical need for transportation? We can help arrange for you to get to the doctor, pick up prescriptions, get to physical therapy, or pick up grocer­ies.
If you have a need or willing to give an occasional ride, contact Pastor Rob.
