Emergency Information

EMERGENCY FORMS

SCF developed an “In Case of Emergency Contact Form” to assist the church if contacted, who to contact in your extended family in case of an emergency.
All information is confidential.
If you do not have a form and would like one, contact Pastor Rob at the Church Office.
You can either pick up a copy at the church or have him send one as an e-mail attachment.

MEDICAL EQUIPMENT:
We have a storage locker of used medical equipment for loan to members and friends.
If you have medical equipment needs, please contact Gina Rosbrook at 541-593-2663 or ginarosbrook1@gmail.com.

MEDICAL TRANSPORTATION NEEDS:
Have a medical need for transportation? We can help arrange for you to get to the doctor, pick up prescriptions, get to physical therapy, or pick up groceries.
If you have a need or willing to give an occasional ride, contact Pastor Rob.
